I ran a couple of more tests on the qemu-system-m68k in UTM and with MacOS 8.1, same site and video.

Audio came out on all outputs even the bluetooth. So its just a qemu-system-ppc screamer issue, me thinks.

I use it to listen to songs and youtube videos on my host mac just fine. It's just UTM that is not working well with any bluetooth output audio, apparently.

I also ran two more tests. I have a dock with extra ports. When I plug a headphone jack on it, no audio too. But if I plug the headphone jack directly on my Mac, there is audio to my headphones. For now, thats the only way to get audio in a private environment without disturbing people around me. If we ignore the dock issue, I think UTM has an issue with outputting audio to bluetooth audio devices.
Have you tried routing through Blackhole? I suspect (but haven't tested yet) that UTM's now using USB Audio, and Bluetooth Audio isn't presented as a USB Audio device. But you should be able to redirect USB audio (or UTM audio) out via Blackhole to your Bluetooth device of choice. I always use Blackhole when I don't get the audio I'm expecting from an app.

no, I have never heard of blackhole.

but honestly I dont want to mess with my current audio configuration because it now works fine with microsoft teams for my current remote work.

The new hearing aid with direct bluetooth is the main reason why I got it, because the old one was really outdated and needed a bluetooh microphone to be able to pass audio to my hearing aid for the teams calls and it frequently had issues. now I no longer have these issues on microsoft teams... I think its just an issue with UTM's PPC screamer, the other archs work fine.

The last screamer source code compiled on qemu 9.1. It will not compile on Qemu 10. So if UTM has been updated to that I assume the screamer will no longer work.
However, recently -device usb-audio has been fixed for Mac OS 9.x so perhaps (when/if UTM supports USB for Mac OS 9.x?) you get your sound back when it is updated to Qemu 10.2.
